چکیده

∗ Corresponding author. E-mail address: [email protected] (M. Costa). therapeutic procedures have not yet been established for bleeding from duodenal varices. Endoscopic band ligation, sclerotherapy, transjugular intrahepatic portosystemic shunt, balloon-occluded retrograde transvenous obliteration, and resection of a segment of bleeding site have been used for treatment.2--5 We present a case of a 48-year-old man with a known history of liver cirrhosis secondary to chronic hepatitis C infection. He was admitted in emergency department due to hematemesis and melena for 24 h. He was hemodynamically stable, and had a hemoglobin level of 7.1 g/dL. The patient was treated with intravenous (IV) pantoprazole, IV octreotide and IV cetfriaxone. Emergency esophagogastroduodenoscopy revealed small esophageal varices without stigmata of hemorrhage. Apart frommild portal gastropathy, no other gastric lesions suggestive of bleeding were noted. Finally, a duodenal varix with a fibrin plug at the second portion, approximately 1 cm in diameter, was found (Fig. 1). N-butyl-2-cyanoacrylate (histoacryl) was injected to the duodenal varix, and bleeding was successfully controlled. Subsequently, the patient remained hemodynamically stable, with no recurrent bleeding. Six months later, a follow-up endoscopy showed large esophageal varices, which were treated with endoscopic band ligation, and severe hypertensive gastropathy. There were not any signs of duodenal varices (Fig. 2).

Ruptured Duodenal Varices Successfully Managed by Endoscopic N-butyl-2-cyanoacrylate Injection

Bleeding from ectopic varices is rare and accounts for only 1% and 5% of all variceal bleeding. However, once the bleeding starts, it becomes difficult to control and is sometimes fatal. We faced a 65-year-old man with ruptured duodenal varices and injected N-butyl-2-cyanoacrylate into the spurting duodenal varices. As a result, oozing was successfully controlled. Subsequently, the patient rema...

Endoscopic management of refractory gastrointestinal non-variceal bleeding using Histoacryl (N-butyl-2-cyanoacrylate) glue

BACKGROUND Histoacryl glue (N-butyl-2-cyanoacrylate) has well-established utility in the endoscopic management of gastrointestinal variceal bleeding. The role of Histoacryl glue in non-variceal bleeding is less clear, and there are few articles describing its use in this setting. METHODS Six patients with intractable non-variceal gastrointestinal bleeding were managed using injection of Histo...

Detection of an Infected N-butyl-2-cyanoacrylate Plug by F-18 FDG PET/CT Scan in a Patient Who Received Endoscopic Intervention for Gastric Variceal Bleeding

Injection of N-butyl-2-cyanoacrylate has been used successfully for treatment of gastric variceal bleeding. Bacteremia after injection of N-butyl-2-cyanoacrylate is well known, however, the method for diagnosis of infected endovascular injected material has remained uncertain. This is the first case reporting use of F-18 FDG PET/CT in detection of the source of infection after control of endosc...

Results of treatment of esophageal variceal hemorrhage with endoscopic injection of n-butyl-2-cyanoacrylate in patients with Child-Pugh class C cirrhosis

BACKGROUND AND STUDY AIMS The results of endoscopic treatment with elastic band ligation for esophageal variceal bleeding in patients with Child-Pugh class C cirrhosis remain poor. In comparison with treatment with elastic band ligation, we have found lower rates of rebleeding and mortality with n-butyl-2-cyanoacrylate injections. Thus, the aim of the current study was to describe our unit's 10...

Safe and successful endoscopic initial treatment and long term eradication of gastric varices by EUS-guided Histoacryl (N-butyl-2-cyanoacrylate) injection

OBJECTIVE: Optimal endoscopic treatment of gastric varices is still not standardized nowadays. Actively bleeding varices may prohibit a successful endoscopic injection therapy of Histoacryl® (N-butyl-2-cyanoacrylate). Since 2006, we have treated gastric varices by standardized endoscopic ultrasound (EUS) guided Histoacryl injection therapy without severe adverse events. MATERIAL AND METHODS: We...
